Cash and self-pay

Understand cash pricing, self-pay estimates, what may be bundled, and how to compare cash with insurance without assuming one is always cheaper.

Cash price basics

A cash or self-pay price is the amount a provider offers when care is not billed through insurance. It may be lower than a billed charge, but what it includes varies.

Before choosing cash

  • Confirm exactly what the price includes
  • Ask whether professional and facility components are separate
  • Ask about labs, pathology, anesthesia, imaging interpretation, supplies, and follow-up
  • Understand cancellation, deposit, and refund policies
  • Ask whether payment is due before service
  • If insured, ask whether cash payment will count toward plan deductible or out-of-pocket limits; it may not

Compare cash and insurance

Cash may fit

When the cash price is clear, affordable, and the customer understands the tradeoffs.

Insurance may fit

When plan-negotiated rates, deductible credit, or coverage make the insured pathway more valuable.

More information needed

When the service, network status, or included billers are uncertain.
